Most people have been taught to treat pain as a warning sign that something is damaged.
Your knee hurts during a squat, so you assume you're wearing something down. Your shoulder aches during a press, so you assume you've injured it. The more pain you feel, the more serious the damage must be.
It seems logical. Pain exists to tell us something is wrong, so surely more pain means more injury. The problem is that the relationship between pain and tissue damage is far more complicated than that.
Pain and damage are related, but they are not perfectly linked. Sometimes significant tissue damage produces very little pain. Other times relatively minor tissue irritation can feel surprisingly severe.
Understanding that difference can completely change how you approach training, rehabilitation, and recovery.
What pain actually is
Pain is produced by the nervous system as a protective response.
When your body detects a potential threat, signals are sent to the brain. The brain then decides whether a pain response is necessary.
That decision is influenced by far more than the condition of the tissue itself. Previous injuries, stress, sleep quality, fatigue, training load, fear of movement, and even your expectations can all influence how much pain you experience.
This is why two people with the same injury can report completely different levels of pain. It is also why the same person can experience different pain levels from the same issue on different days.
The tissue matters, but it is only one piece of the puzzle.
What we see on scans
One of the clearest examples of this comes from medical imaging.
Research has consistently found structural changes in people who have no pain at all.
Pain-free adults commonly show findings such as disc bulges, meniscus tears, rotator cuff tears, and tendon degeneration. Many people are walking around with these changes every day without knowing they exist. If tissue damage was the only driver of pain, every one of these people would be experiencing symptoms.
On the other hand, some people report significant pain despite relatively minor findings on imaging. This doesn't mean the pain isn't real. It highlights that pain cannot be explained by tissue structure alone.
Why this matters for training
One of the biggest mistakes people make is assuming that any pain during exercise means damage is occurring.
In some situations, pain absolutely deserves attention. Sudden injuries, rapid swelling, loss of function, or pain following trauma should never be ignored. But many persistent issues don't behave this way. Tendon pain is a good example.
Someone with patellar tendinopathy might feel discomfort during squats, lunges, or jumping, yet the tendon may still be capable of tolerating and adapting to those loads. If that person stops every activity that causes discomfort, their strength declines, their tendon capacity decreases, and returning to sport often becomes harder rather than easier.
This is one of the reasons modern rehabilitation focuses heavily on load management. The goal is to find a level of loading the tissue can tolerate and gradually build capacity over time.
Pain can become more sensitive
Think of pain like an alarm system. An alarm is useful when there is a genuine threat. But alarm systems can also become sensitive. A gust of wind, a passing car, or a loose window can trigger the same response that was designed for an actual break-in.
The pain system can behave similarly.
After an injury, or during periods of high stress and fatigue, the body can become more protective. Activities that were previously tolerated may begin producing discomfort despite no meaningful change in tissue health.
This doesn't mean you should ignore pain, It means the presence of pain does not automatically tell you how much damage exists.

The amount of pain you feel does not always reflect the amount of tissue damage present. Understanding that distinction can help you make better decisions when training, recovering from injury, or dealing with persistent aches and pains.
The goal is not to fear pain, and it is not to ignore it. The goal is to understand what it is actually telling you. Sometimes the smartest thing you can do is keep moving, even when the alarm is making a little noise.
